You will be asked to complete a confidential client questionnaire so we can better assist you with your weight loss goal. Your weight will also be recorded on your take-home card so you can track your achievement while on the program. Your weight details will also be entered into our online database, so we can better manage your records.

We will also offer to take and keep a “before” photo of you upon joining. The “before” photo is for your personal record only. And of course later, there’s the “after” photo too. We recommend that you do go through this process as is a great way for you to monitor your progress and really see the results.
